Job Description:

The Lead Family Advocate is responsible for the efficient and effective provision of programmatic and clinical care at CityHouse. The Lead Family Advocate position encompasses not only micro-level clinical care but engages in family and community systems with the goal of creating sustainable growth in each of our residents. CityHouse Residential and Alumni Programs go beyond the provision of affordable housing and seek to build a community of compassion, accountability, and development; It is the Lead Family Advocate's role to ensure the program is meeting the evolving needs of the families we serve.CityHouse Program consists of long term residential housing, case management, weekly clinical sessions with Family Advocates, treatment planning and goal identification, financial literacy coaching, parenting classes, career support, and discharge planning. The Lead Family Advocate has the responsibility to lead a small team in ensuring the resident's time in the program is used to work on both concrete and clinical goals that aid in the creation of each mother's sustainable life after our 2-year program. The Lead Family Advocate is responsible for managing program staff as well as social work BSW and MSW interns. Must have certification for Agency Field Instructor. ESSENTIAL DUTIES AND RESPONSIBILITES:Intake Coordination:Review incoming applications for open residential spots in CityHouse ProgramAssess applicants level of functioning, insight, and ability to engage in CityHouse Residential program and make recommendations for placement or referralAct as the main point of contact for resident application screening, interview, and placement.Coordinate and schedule interviews with CityHouse Staff for collaborative review of application and interview with the applicant.Place applicants in open spots based on family size, child ages, and knowledge of the CityHouse program.Welcome residents to campus, assist in lease signing, and onboarding of new residents.Ensure resident is connected to support staff and their main family advocate for immediate start of services.Performs comprehensive assessment and goal planning:Gather a comprehensive history and clinical conception of the resident through intake paperwork including BioPsychoSocial documents, treatment plans, and additional assessments.Collaborate with the referent, community agencies, and CityHouse staff to assess the resident's presentation and progress.Formulate an accurate diagnostic impression of each resident and use this to inform treatment goals and objectives as well as referrals to outside resources like psychiatry, psychological testing, and substance use treatment.Prioritize information from assessments into goals and measurable objectives.Work with the resident in identifying personal strengths and assets.Establish appropriate criteria to evaluate progress. Track progress with treatment plan reviews, being mindful to adapt treatment plans as the resident's needs change.Communicates rights and expectations for behavior to families in a consistent and respectful manner.Conducts individual group and family therapy:Establishes therapeutic relationships based upon respect, genuineness and empathy with individuals and groups.Utilizes active listening and motivational techniques to support residents as they identify goals and meet objectives.Meet weekly with residents to ensure continual and comprehensive clinical care.Facilitate weekly Family Advocate Meetings to coordinate care with social workers and case managers.Engage with residents with strong boundaries, sensitive to the nature of the work that requires both therapeutic and authoritative roles.Shows respect for the resident's individual position and approach individual differences with curiosity and compassion.Effective and timely communication with families.Completes accurate and timely documentation:Documents in compliance with the required content, in a timely matter.Records assessment data and progress information in an objective, concise manner.Maintains the security of clinical records.Conducts comprehensive biopsychosocial client assessments.COMPETENCIES:To perform the job successfully, an individual should demonstrate the following competencies:IntellectualAnalytical - Synthesizes complex or diverse information; Proof of current training in verbal de-escalation techniques.Demonstrates the ability to participate in program development and evaluation.Demonstrates the ability to collect and compile data as required by program and funding applications.OTHER QUALIFICATIONS:Must be a Licensed Clinical Social Worker in the state of Florida and have a minimum of a Master's Degree in one or more of the following: social work, mental health counseling, family therapyMinimum of two years of experience working with women and children with a trauma history.Demonstrates knowledge of Trauma-Informed Care specific interventions.Demonstrates knowledge of Palm Beach County areas and resources.Successfully complete a criminal background screening and drug screening.
