The Case Manager functions as the liaison between physician, patient and family, multidisciplinary treatment team, Managed Care plans, and when indicated, legal representative(s). The Case Manager facilitates delivery of clinical and community services to patients and families through effective management of available resources. The Case Manager ensures the appropriateness and cost effectiveness of treatment. Facilitates decision making in establishing an evaluation program, an interdisciplinary treatment plan, and an assessment of the patient's status and need for provision of continuing care. The Case Manager formulates a specific discharge plan for the expected outcomes and specific discharge date. The Case Manager establishes helping relationships and advocates for needed resources and services for the patient.

Qualifications - External

POSITION QUALIFICATIONS:

EDUCATION:

Minimum: Education recognized by the State of Florida as qualification for Registered Nurse licensure.

Preferred: Bachelor's degree in Nursing

EXPERIENCE:

Minimum: Five (5) years of clinical experience in acute trauma care.

REQUIRED CERTIFICATION/LICENSURE/REGISTRATION:

Florida Registered Nurse license and AHA BLS Certification required.

Certification in Critical Care, Emergency Nursing, and Case Management preferred.

OTHER QUALIFICATIONS:

• Knowledge of Joint Commission Standards, Quality Improvement Process and Risk Management
• 
  Knowledge of Infection Control Surveillance and Universal Precautions
• 
  Knowledge of Florida State Department of Health SATC/SAPTRC Approval

  Knowledge of Palm Beach County Health Care District Trauma Agency

• Active participant in continuing education and professional groups in order to remain current in practice and changes in regulations
